The Professional Practice Apprentice Coder is considered an on the job training position that reviews, analyzes and codes diagnoses and procedures information that uses ICD-10 CM and CPT-4.
The coding function is a primary source for data and information used in health care today, and promotes provider/patient continuity, accurate database information and the ability to optimize reimbursement.
The coding function also ensures compliance with established coding guidelines, third party reimbursement policies, regulations and accreditation guidelines.
Responsibilities:
Determine the proper codes and modifiers for all billable services utilizing CPT-4 and ICD-10-CM code sets and assign diagnosis and procedures to medical records in accordance with coding compliance with established guidelines.
Code for 100% of medical records/encounters for each assigned clinic or encounter type within five working days following completion of documentation with 97% accuracy as demonstrated by coding audits and or observed by the manager Alerts coding leads, supervisor, manager of coding servces or director of HIM in identifiying and analzying problems or issues that prevent the accurate and timely coding of medical records.
Maintain credentials/certification and education to stay current with all coding skills to accurately abstract and code medical records.
Practice time management to ensure that all charges are entered and all edits are handled prior to month end.
