ResponsibilitiesThe RoleThe Care Integration Team Divisional Manager is responsible for managing a team of licensed social workers, nurses, and community health workers all collaborating to deliver a whole person health approach to patient care.
The Divisional Manager is responsible for providing clinical guidance, supervision, and overarching direction to associates on the team, as well as ensuring key performance metrics are being driven toward and communicating those results with leadership.
The Manager will utilize advanced clinical acumen, experience, and knowledge to lead this multidisciplinary team.
As a guideline, this role involves spending 70% of the time on operational excellence and program delivery, 30% on relationships.
Major Duties and Responsibilities Oversees day-to-day operations, recruiting/hiring, team management, and overall performance for Divisional pool.
Enacts quality review process to ensure proper clinical care is being provided and documented accordingly.
Ensures clinical program integrity and addresses performance and program improvement opportunities, escalating to Divisional Director as appropriate.
Provide clinical education and training to team as needed.
Support team in case consultation and clinical escalations as needed.
Engages with leadership at the market and national level to report out on team metrics and progress and receive feedback.
Accountability to key population health metrics, including quality, utilization, and financial measures.
Required Qualifications Registered Nurse (RN) License At least 5 years post undergraduate experience in clinical nursing work or clinical social work Prior experience working with patients with behavioral health conditions.
At least 2 years of team management experience Preferred Qualifications Prior experience working with older adults.
Clinical competence in working with patients with Serious Mental Illness and in crisis intervention.
Familiarity with and knowledge of chronic conditions, disease management, and navigation of medical systems of care Bilingual in English/Spanish with the ability to speak, read and write in both languages without limitations and assistanceCharacteristics of the qualified candidate:
Advanced clinical acumen including assessment, diagnostic, therapeutic and crisis intervention skills as well as expert knowledge of behavioral health resources.
Excellent communication skills, including follow-through communication and the ability to interpret and translate data to tell a story via executive level presentations.
Relationship and team building skills.
A passionate advocate for improving clinician and patient experience through population health management.
Capable of setting SMART goals, aligned with organization, and holding staff accountable for achieving goals.
Relationship management and negotiation skills to ensure key organizational and community partners feel engaged, heard, and respected.
Pro-active, positive attitude, and comfortable being a change agent.
Ability to lead and facilitate meetings with MDs, Operations Leaders and across a matrixed organization.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
